Whanganui Regional Health Network (WRHN) is a Primary Health Organisation, covering a population of 60,000 residing in urban and rural locations. As a team we are passionate about health and equitable care delivery for the Whanganui community.
This is a 1FTE role for a registered pharmacist (with a current practising certificate) who has a genuine interest in reducing inequities for Maori and other vulnerable populations, and working with priority populations in improving health outcomes. Effective engagement and the ability to build and maintain positive relationships with our stakeholders is vital to the success of this position.
The successful applicant will play an integral member of primary care interdisciplinary teams, supporting WRHN general practice members and enrolled population. The primary function focusing on;
• Supporting primary care providers to address inequities in prescribing and utilisation of medications
• Clinical medication reviews including virtual advice and consultation focused on high risk, polypharmacy and priority populations
• Medicines management support including;
- Post-acute discharge referred patients identified as being at risk of adverse drug events (ADEs) and re- admission
- Clinics for high risk and priority populations
- General practice support
- Consumer support and patient education
• Continuous quality improvement through regular and structured review processes focusing on best practice prescribing and testing
• Assisting with peer review and other educational activities – this includes facilitation and provision of resources and education
• Providing evidence-based / best practice medicines information to primary care practitioners.
• Timely analysis and interpretation of medication utilisation trends for general practice prescribers
